Make recording incidents, injuries and illnesses easy with ECC’s Incident, Injury and Illness Register.

The ECC Incident, Injury and Illness Register is a pad with detachable duplicate pages, created specifically for ECE centres in New Zealand. It provides a practical and consistent way to record incidents, injuries, illnesses and near misses involving children, employees, contractors, volunteers and visitors.

The updated form brings the information centres may need into one place, including what happened, the type and location of any injury or health issue, actions taken, parent or guardian notification, relevant agency notifications, investigations and follow-up actions.

The Register can be used to record minor incidents, injuries and illnesses as well as more serious events that may require notification to the Ministry of Education or another relevant agency. ECE centres are required to keep records of incidents, injuries and illnesses, including minor events.

Each form is completed in duplicate. The blue copy is retained in the centre’s Incident, Injury and Illness Register, while the white copy can be provided to the appropriate person or agency. For incidents involving a child, the white copy is provided to the child’s parent or guardian.
